The Center for Community Health Navigation at NewYork-Presbyterian Hospital is dedicated to supporting the health and well-being of patients through the delivery of culturally-competent, peer-based support in the emergency department, inpatient, outpatient, and community settings with the ultimate goals to promote healthcare self-management and to decrease avoidable system utilization.
